The premise

The hardest part of home wound care is the car ride.

A homebound patient with a complex wound is caught between two systems. The home health nurse sees the wound weekly and can describe exactly how it is changing, but cannot write the orders. The wound care physician can write the orders, but only sees the patient when someone arranges a stretcher van, a family member takes the day off, and the patient spends four hours away from home for a fifteen-minute look.

So the plan lags behind the wound. Dressings continue longer than they should. A referral that needed making in week two gets made in week six.

AFRA closes that gap by putting the physician in the house. Evaluation and treatment happen at the bedside, with the home health nurse present where possible, and the orders are written on the spot. The patient does not travel at all.
