The premise

Your wound nurse doesn’t need replacing. She needs orders.

Most skilled nursing buildings already have someone who knows wounds. She measures, she stages, she knows which resident’s heel is heading the wrong way. What she does not have is a physician who can see the wound in front of her and write against it the same day.

So the wound waits on a consult. Or it goes out to a clinic, and comes back four days later with a note written by someone who saw it once, does not know the resident, and will not see it again. Meanwhile the floor keeps doing what the last order said.

AFRA rounds inside the building on a set cadence. The physician assesses at the bedside with your staff present, adjusts the plan on the spot, and writes documentation into your chart rather than into a separate system. When surveyors ask what changed and when, the answer is in one place.

A wound followed by the same physician for three months is a different clinical picture than one seen three times by three people.

Short-stay and long-stay residents need different things from a wound service. The same visit covers both.

  • Short-stay rehabilitationPost-surgical incisions that dehisce, wounds arriving with the admission, and pressure injuries acquired before the resident reached you. Fast assessment matters here because the clock is short.
  • Long-stay residentsChronic wounds followed over months, where the value is continuity: the same physician, the same judgment, measured against a consistent baseline.
  • Long-term care facilitiesComplex and slow-healing wounds in medically fragile residents, including palliative and comfort-focused management where healing is not the realistic goal.
  • Your wound nurseWe work alongside facility wound care staff rather than around them, and train the aides who change dressings between visits.
